Low progesterone causes miscarriage, infertility – Expert

Adaora Ukoh, an Abuja based gynaecologist, has said that low progesterone could lead to miscarriage and infertility.
Ms. Ukoh told the News Agency of Nigeria on Monday in Abuja that if a woman doesn’t have enough progesterone, it would be difficult for pregnancy to occur.
She described progesterone as one of the hormones in a woman’s body that stimulates and regulates various functions such as the ovulation and menstrual cycle.
The expert also explained progesterone as a female sex hormone produced mainly in the ovaries following ovulation each month which is crucial in menstrual cycle as well as maintaining pregnancy.
According to her, progesterone is one of the steroid hormones secreted by the corpus luteum, a temporary endocrine gland that the body produces after ovulation during the second half of a woman’s menstrual cycle.
She added that fertility and menstruation are largely controlled by hormones such as progesterone and if it becomes low, it could reduce the chances of having a normal menstrual cycle as such leads to infertility.
“Women who have low progesterone will have abnormal menstrual cycles and also have difficulty conceiving because the progesterone does not trigger the proper environment for a conceived egg to grow.
“Those that succeed in getting pregnant are at higher risk of miscarriage or pre-term delivery because the hormone progesterone helps maintain pregnancy.

“If you have low progesterone it will be difficult to become pregnant as well as staying pregnant,’’ she said.
However, Ms. Ukoh said that treating the condition could start from introducing hormone therapy in order to increase progesterone as well as thicken the uterine lining.
She suggested oral medications, creams and gels that could be used in the vagina in order to help promote the level of progesterone.
Ms. Ukoh also recommended the intake of vitamins such as B and C as well as consuming foods rich in zinc such as shellfish to help treat the condition.
She further enjoined women with the condition to avoid stress, noting that cortisol are released when stressed, which reduces the level of progesterone.
The expert however advised women trying to conceive who have liver disease and blood clots among others, to avoid the use of hormone therapy.

Fertile cervical mucus is a clue that ovulation is coming. If you want to get pregnant, you need to have sex before ovulation.

You can predict this by checking and tracking cervical mucus changes.

Yes, this will mean looking and (usually) touching your vaginal discharge. You may feel squeamish about it, but… this is your body!

Knowing how your body works can be really empowering. It’s been years since I was actively trying to conceive. And yet, even now, I am always aware of where I am in my cycle and when I’m about to ovulate.

What Is Cervical Mucus?

Cervical mucus is secreted by glands found in and around the cervix. Hormonal changes throughout a woman’s reproductive cycle change the amount and consistency of this mucus. Cervical mucus may also be called cervical fluids.

The job of cervical mucus is to either…

prevent things from entering the uterus through the cervix, or
to nourish and help transport sperm through the cervix into the uterus

Just before ovulation occurs, the hormone estrogen causes an increase in cervical mucus and changes it into a stretchy, viscous like substance. This helps sperm survive and swim.

After ovulation, the hormone progesterone causes cervical mucus to become sticky and thick. This stops sperm (and any other foreign substance) from getting through to the uterus.
Understanding the Connection Between Ovulation and Cervical Mucus

If you already track your body basal temperature (BBT), adding cervical mucus tracking is a good idea.

Your BBT will tell you if and when you ovulated after it happened. Cervical mucus changes can tell you before you ovulate.

As ovulation approaches, your cervical mucus changes from a consistency that’s not sperm friendly to a more fertile variety.

While everyone’s body is different, the general changes that cervical mucus go through are…
dry or sticky
to creamy, like lotion
to wet and watery
to a raw egg white consistency
then, back to dry and sticky

When your cervical mucus is in the wet or raw-egg-white consistency stage, ovulation is approaching. This is the best time to have sex if you want to get pregnant.

How to Check Your Cervical Mucus:

First, wash and dry your hands well.

Find a comfortable position, either by sitting on the toilet, squatting, or standing up and putting one leg up on the bathtub edge or toilet seat.

Reach one finger inside your vagina; your index or middle finger is probably best. (Be careful not to scratch yourself.)
Depending on how much cervical mucus you’re producing, you may not need to reach so far, but getting a sample from near your cervix is ideal.

Remove your finger from your vagina and observe the consistency of whatever mucus you find.

Do this by both looking at the mucus and rolling what you find between two fingers (usually your thumb and index finger). Try pressing your fingers together and then slowly moving them apart.

If what you find seems sticky, or findings are scant, you’re probably not ovulating yet.
If what you find is creamy, ovulation may be coming, but not just yet.
If what you find is wet, watery, and slightly stretchy, ovulation is very likely close. Find time for some baby-making sex.
If what you find is very wet, stretches between your fingers for an inch or more, and resembles raw egg white, your cervical mucus is very fertile. Ovulation is right around the corner, and now is the ideal time for intercourse.

If you are charting your BBT, you should mark down on your chart your cervical mucus findings.

Abbreviations often used are S for sticky, C for creamy, W for wet, and EW (or EWCM) for egg-white cervical mucus.

Tips for Checking Cervical Mucus

Don’t check your cervical mucus during or right after sex.

Also, avoid checking when you’re feeling sexually aroused.

Arousal fluids are not the same as fertile cervical mucus, but you probably won’t be able to tell the difference. So don’t check at these times.

Checking after sex is also a bad idea. It’s too easy to confuse semen for cervical secretions.

You can check your cervical mucus by looking at the toilet paper or your underwear.

Not everyone is comfortable with putting their finger inside to check themselves. But you don’t have to.

You can instead pay attention to how wet your vulva feels on a day to day basis, pay attention to the discharge on your underwear, or look at the toilet paper after urination.

There is, in fact, an entire method based on this: the Billings Ovulation Method.

With all that said, it’s easier for many women to physically check themselves and reach inside.

Consider checking your cervical mucus after bowel movements.

Of course, first wash your hands well! But if you have trouble finding cervical mucus, it may be easier after a bowel movement.

Having a bowel movement moves down any vaginal discharge closer to the entrance of your vagina.

If you have multiple patches of fertile cervical mucus, look for additional ovulation signs beside CM.

Some women, especially those with PCOS, have several patches of fertile-looking cervical mucus throughout their cycle.

If this is your situation, predicting ovulation by tracking cervical mucus might not work well for you.

You should consider paying attention to other ovulation signs, like cervical position changes.

You may also want to chart your basal body temperature, so you know which (if any) of the fertile cervical fluids indicated ovulation.

Some medication may interfere with your cervical fluids.

Antihistamines dry up more than your sinuses — they also dry up your cervical fluids.

Ironically, Clomid can prevent you from having fertile quality cervical mucus. In this case, you might not find as much fertile cervical mucus before ovulation.

In this case, you might want to try using an ovulation predictor kit to detect ovulation instead.

If you never get fertile quality cervical mucus, tell your doctor.

Lacking fertile quality cervical mucus can be a sign of a hormonal imbalance or fertility problem.

Cervical mucus that doesn’t ever become fertile is sometimes referred to as hostile cervical mucus.

You may see fertile cervical mucus again right before your period.

Some women notice that their cervical mucus becomes wet or almost egg-white like again right before menstruation.

Obviously, this isn’t a sign of impending ovulation.

Women sometimes wonder if getting a lot of cervical mucus just before their period is due is a possible early pregnancy sign.

The fact of the matter is, it’s almost impossible to tell the difference between “early pregnancy” cervical mucus and regular “just before your period comes” cervical mucus.

You may confuse semen with cervical mucus.

A day or two after sexual intercourse, you may confuse semen with wet cervical mucus.

With experience, you can learn how to differentiate the two.

But for the purposes of getting pregnant, assume that you may be approaching ovulation and mark your calendar or chart accordingly.

Do not attempt to wash away your natural vaginal fluids!

Cervical mucus is normal and healthy. Some women wash away “ovulation secretions” thinking they are unhygienic or unhealthy.

Don’t do this! Douching can actually decrease your fertility.

This is Victor, I am a Consultant Clinical Specialist Radiographer[Ultrasounds], with over 15 years post qualification experience, I have worked as a sonographer in Nigeria for over 14 years, presently I live and work in The Republic of Ireland [though temporarily]as a sonographer

My topic today is for the general online community, but specially for the Nigerian online community, because this is an issue that affect us seriously.
Today I will be writing about fibroids. Uterine fibroids are benign tumours, which grow from the muscle layers of the womb. They are the most common benign tumours in female, often asymptomatic, they cause symptoms in about 25% of white and 50% of black women.
Fibroids are named according to where they are found. There are four types
[1] INTRAMURAL FIBROIDS are found in the walls of the womb and are the most common type of fibroids.
[2] SUBSEROSAL FIBROIDS are found growing outside the walls of the womb and can become very large. They can also grow on stalks [called pendunculate fibroids]
[3] SUBMUCOSAL FIBROIDS are found in the muscle beneath the inner lining of the womb walls
[4] CERVICAL FIBROIDS are found in the walls of the cervix [neck of the wombs].

In rare cases, fibroids can turn cancerous

SYMPTOMS

Symptoms relate to the location of the fibroids and its size. Important symptoms include abnormal gynaecologic haemorrhage[bleeding], heavy or/and painful periods, abdominal discomfort or bloating, back ache, urinary frequency or retention, and in some cases infertility. There may also be pain during intercourse depending on the location of the fibroids.
During pregnancy they maybe the cause of miscarriage, bleeding, premature labour, or interference with the position of the fetus.



DIAGNOSIS

Diagnosis is usually accomplished by bimanual examination, better yet by gynaecologic ultrasonography[SCAN], magnetic resonance imaging[MRI] and laparascopy. Fibroids can also be diagnosed during HSG examination.



TREATMENTS

[1] Surgery [a] hysteroscopic myomectomy [b] laparoscopic myomectomy [c] laparotomic myomectomy.
[2] Uterine artery Embolization [UAE]
[3] Magnetic Resonance Guided Focused Ultrasound [HIFU]



Fibroids are very serious issues in Nigeria where I practiced for over 14years. Fibroids became a real issue in Nigeria after the death of popular MEE Mofe-Damijo in 1996 from post surgical complications from removal of fibroids. I was a very young sonographer at that time and I attended to thousands of women and up till this day fibroids is still a big issue.
I have being opportuned to have travelled to 4 continents in the world [Africa, Asia, Middle East, and Europe] and worked in Africa and Europe as a specialist sonographer, fibroids still remain an issue though much more in Africa. It is also a big business, good pay for the surgeon and also good money to traditional healers.
I have no problems with traditional healers and alternative medical practitioners, but what I believe is that some of them should give true information to the patients.
I have attended to so many women in Nigeria who have visited some of these alternative medical practitioners and they claimed to be able to give them some concoction that will make these women to excrete these fibroids, to me this is not fair , for years I have followed these women and I must confess non received any so called cure.
I am not against anyone taking any form of cure she wants whether orthodox or traditional, but safety should come first, NAFDAC must confirm that these concoction are safe for human consumption.
I also noticed that some of the concoction given to these women with fibroids/infertility problems are the same ones given to women with general infertility problems. From the numerous scans I did on this issue in Nigeria I noticed that their ovaries produce multiple mature follicles than usual, which shows that these drugs contain some natural hormonal substances that stimulate the ovaries and help the infertile women to get pregnant.
If such drugs are given to women with fibroids, these could lead to increase in the size of the fibroids. While these women are looking to reduce the size of the fibroids, the administered concoction increases the size [Please be careful]. These concoction have also being known to lead to kidney failure.

I want to advise clearly that any woman with fibroids should not get stressed, if there is no threatening symptoms, be relaxed, don’t go running up and down and get duped.
If you are an infertility patient with known fibroids, don’t always assume that the fibroids are the problem, do tests to checkout your fallopian tubes, ovarian functions, sperm tests etc [Ask your gynaecologist for further information]. If you are having excessive bleeding or the fibroids are very large make sure you see your doctor, surgery is best in this instance, because the fibroids can twist, or compress the ureters and give renal problems.

I want to specially talk to Consultant radiologists in Nigeria, Uterine Artery Embolization[UAE] is relatively cheap and very effective treatment for fibroids. It only needs an X-ray fluoroscopy unit, which is available in some centres in Nigeria.
I have known women in Nigeria who paid as much as N200000 for traditional cure of fibroids and they are getting no results.
Uterine Artery Embolization is known to have worked , I have attended to some women in Europe that have done it and have seen the fibroid masses shrink, saving the women the complication of surgery.
I recommend that Nigerian radiologists should come to Europe and learn these skills, the Nigerian women need it, they will be saved from the complication of unnecessary surgeries, and drinking of strange concoctions from some strange healers .

Feeling pregnant? If you've been trying to conceive for awhile, this may be a monthly issue for you. You feel pregnant. You have all these signs and symptoms of pregnancy. Fatigue, cravings, maybe even nausea.

But then, when your period arrives, you discover that nope... not pregnant this month either.

The experience of feeling pregnant when trying to conceive isn't uncommon. Spend any time on fertility forums or on social media sites, and you're bound to hear members refer to "imaginary pregnancy symptoms."


Are these feelings all in your head?

Imaginary Pregnancy Symptoms


Imaginary Pregnancy Symptoms (IPS) are exactly what they sound like - symptoms women experience that make them think that they may be pregnant.

Don't expect to hear your doctor use the term IPS. It's not a technical term.

The phrase was invented by fertility-challenged as a loving way to refer to those obnoxious "symptoms" that haunt you during the two-week wait.

The time between ovulation and your expected period is when you're most likely to be anxious about whether or not this month will be the month.

It's natural to assume that you may imagine some early pregnancy symptoms, such as tender breasts, fatigue, bloating, emotional sensitivity, light cramping and even food cravings.

Much like an oasis in the desert, you want to be pregnant so much that you're sure you can feel it.

Your Optimistic Body and Progesterone


What may surprise you is that these feelings aren't all in your head.

They're real reactions to the hormones in your body that are preparing for possible pregnancy.

Our bodies are optimistic when it comes to pregnancy potential.

As soon as ovulation occurs, the body starts preparing for a new life. This occurs even if conception did not take place.

One of the hormones responsible for maintaining a healthy early pregnancy is progesterone.


Progesterone increases just after ovulation. One of its many roles is to support a potential embryo. If you're not pregnant, your progesterone levels will drop after 12 to 16 days after ovulation. This drop brings on your period.

High levels of progesterone can make you feel tired and emotional. This hormone is also responsible for tender breasts, constipation, and fluid retention.

Fatigue, moodiness, tender breasts... sound like early pregnancy symptoms, right?

Progesterone levels will rise in your body whether or not you are pregnant.

Also, fertility drug side effects can sometimes be mistaken for early pregnancy symptoms. This is especially true if you're taking progesterone injections or suppositories.

But What About Women Who Swear They Just Knew...?


We all know at least one person with a "feeling pregnant" story that came true.

They just knew that month was different.

Maybe one particular symptom was stronger. (They say...) They were extra tired. Or they were craving some food they never eat otherwise.


Or they had a strange cramp or twinge.

They may claim women's intuition let them know they were with child before the pregnancy test came back positive.

Here's the thing with these kinds of tales: these women are putting much more weight on the one time they felt pregnant and actually were pregnant over the dozens of times those same feelings didn't indicate pregnancy.

It's called confirmation bias.

It's a nice idea that a woman can "just know" when she's pregnant. But there's no research to back up these stories.

The Bottom Line on Feeling Pregnant


If we could just feel whether we're pregnant or not, it sure would cut down the anxiety of the two-week wait!

The symptoms of early pregnancy, though, are practically indistinguishable from any normal two-week wait symptoms.

While your "pregnant feelings" aren't 100% imagined, you can feel worse the more you focus on the feelings.

It may help to remind yourself that whether you feel pregnant or not, it doesn't mean anything.

Some women are sure they are pregnant, complete with throwing up in the morning, and then find out they're not.

Some women feel absolutely nothing and find out they're pregnant after all.

The only way to know if you're pregnant is to wait until your period is late and take a pregnancy test.

Note: When I talk about imaginary pregnancy symptoms, I'm not referring to the very serious psychological condition pseudocyesis, a psychological condition where a woman really believes that she is pregnant when she isn't. This is completely different than the normal experience of feeling that you may be pregnant, even if you aren't, during the two-week wait.
