THE COLD GRIP OF PREGNANCY CONCERNS


As I am sure many of you already know, I am one person that really love travelling, just to learn new things, take classes and continue to learn. It is funny that someone said many years ago that learning and acquiring knowledge have no end. Since that is my hobby I am in trouble because I might just continue this way forever.

This year I visited Canada, it is actually my first time there, it is one of the most beautiful countries I have travelled to, however what struck me was the cold, the temperatures during the early winter ranges from -12⁰C to 0⁰C. Everywhere is white due to snow, a friend told me that the snow and ice on the ground will not melt till March next year, which will be almost 4 to 5 months, this is something. Imagine walking out in the street without wearing thick coat and sweaters, man you will freeze.

When I think of all the freezing , it makes me think of an infertility treatment where the sperm , egg or fertilized eggs have to be frozen till when it is needed. Some time ago I had this couple that visited our clinic for fertility issues, as usual we decided to run some fertility studies on them, the husband did the all important and simple sperm test, which is basic, he came out in flying colours. So all eyes were on the lady, she went through the many routine and basic tests for females such as, Transvaginal scan, HSG, SIS, Folliculometry, Hormonal profile and some other studies, to the surprise of all she too came through in flying colours.

Was this unexpected, not really, in my many years of practice, many couple who have infertility issues have been known to pass most if not all of the relevant fertility tests. Although I will confess to you this is a challenge to most fertility specialist, but that does not make it an issue. When infertility couples pass all their tests, the major way around it is to try alternative fertility treatment, which could include the simple and time tested TIS [ Timed Intercourse Study], IUI [ Intrauterine Insemination or the popular but expensive IVF [ Invitro Fertilisation].

What is the idea about using these methods to treat couples that have passed all the known fertility tests, for one there is what is called Unexplained Infertility, where specialist cannot really say what is wrong with the client, except that it is universally agreed that the egg and the sperm are not just meeting. The use of these fertility treatments will greatly improve their chances of meeting, and have been known to work in most areas of unexplained infertility, however I must as always add the caveat, that there is no fertility treatment that is guaranteed to work, because no specialist or fertility study is God.

I will end by looking at the most simple of the fertility treatments called TIMED SEXUAL INTERCOURSE STUDY [SIS]. Timed intercourse is the act of having sexual relations on the most fertile days of a woman’s monthly cycle. This is a treatment option for all cases of primary or secondary infertility, in which basic studies find no clear reason for the infertility. It is often used by young couples, patients who have spent little time trying to get pregnant and couples with unexplained infertility who want a simple and natural choice before undergoing more complex techniques.

Ovulation is stimulated with oral medication that increases endogenous FSH and therefore improves the selection and maturation of eggs . Patients are also sometimes given oral oestrogen that improves the ability of cervical mucus to move sperm up the fallopian tubes and also improves endometrial proliferation. Follicular growth is monitored via ultrasound. When it is considered the right time, ovulation is induced with a subcutaneous HCG injection. Couples will have sexual intercourse 24 to 48 hours after administering the HCG.

This method though simple have been known to lead to many pregmancies without moving to the complex and expensive treatments. However after 4 or 5 trials without success, the next level of treatment such as IUI and then IVF should be suggested.
